How China turned to telemedicine during Covid-19 pandemic?

When tech entrepreneur Alvin Foo first moved to Shanghai 15 years ago, his experience of visiting a doctor usually meant joining a long line at a hospital: “The way that people go to hospital in the past was … if you needed to go and see a doctor, the first thing that you need to do is, you need to get a number. And once you get a number, usually you have to queue,” he told CNBC by phone.

“Some people … started queuing the night before,” Foo added.

Foo, co-founder of cryptocurrency investment firm DAOventures, described a time when his wife arrived at a skin hospital at 4 a.m., but by the time she got to the front of the line, all the allocated appointments were gone. “That’s the kind of pain that a lot of Chinese people, the patients (would) have to actually deal with,” he said.

Those with the means might hire someone to wait in line for them, an illegal practice that Chinese authorities have clamped down on, but the combination of a population of 1.4 billion and a lack of medical staff means demand still outstrips supply.

What changed life for Foo and millions of others in China has been the launch of online appointment booking and consultations from the likes of Tencent-backed WeDoctor, which has reported having around 250,000 doctors registered on its platform, and Ping An Good Doctor, which claims to have more than 340 million registered users. During the coronavirus outbreak, these platforms and other giants such as JD Health offered free online consultations, which has accelerated their uptake.

“People can now freely consult a doctor, unlike in the past where it was a pain even just to get a … number to seek consultation,” Foo told CNBC.

While there are still lines and long waiting times for in-person appointments, these pushes into online medicine by tech giants, as well as moves from the state, are set to help the Chinese telehealth market reach $54.2 billion by 2025, according to a UBS estimate (see chart). The firm’s “Future of Humans” report, published in September, estimated its current value as $8.6 billion, and suggested China’s market would outstrip the U.S. market in 2023.

There is huge potential for digitally-provided health care in China partly because of its aging population, according to Carl Berrisford, a strategist at UBS Global Wealth Management’s chief investment office, based in Hong Kong. “The other (reason) is a very elevated level of broadband and internet penetration and mobile penetration. And … the distinct pieces of the actual medical consulting system … you have to go to a hospital if you want to see a doctor,” he told CNBC by phone.

Telemedicine is the ability of healthcare providers to meet with patients remotely via telephone or video.

This practice has been around almost as long as telephones but has become more popular and practical during the COVID-19 pandemic.

Improvements in technology and changes to insurance reimbursement rulings have helped increase the acceptance of telemedicine. Without it, doctors and therapists wouldn’t be able to connect with patients as easily, especially during the pandemic.

As telemedicine has become widespread, professionals and patients have grown more comfortable connecting virtually.

Benefits

Telemedicine isn’t meant to take the place of face-to-face visits. While it does have some disadvantages, which we’ll get into later, its benefits are undeniable.

They include:

1. Easy access to specialists

Not everyone has an ongoing relationship with a doctor they can call when they need one. Many online medical networks offer round-the-clock access to all kinds of specialists, without an appointment, at any time of day or night.

2. Lower cost

Doctors and therapists can be expensive, even for people with good health insurance. Telemedicine appointments typically cost less than in-person visits do. This reduces out-of-pocket costs, removing a barrier to care.

Not having adequate health insurance can be an obstacle to seeing a doctor. Many online companies provide cash-pay telemedicine, which doesn’t require health insurance or referrals.

4. Medical access for people in rural areas

Country living has many benefits, but fast access to medical care isn’t always one of them. For people who live many miles from the nearest medical facility, telemedicine provides a way to meet with a doctor quickly.

This saves time and allows people to stay off the road when driving conditions are less than optimal, such as during a snowstorm or hailstorm.

5. Medical access for people in underserved urban areas

The trend of hospital closures in inner-city neighborhoods has affected thousands of Americans, especially communities of color and people without health insurance.

Telemedicine helps break this cycle by providing a way for people to see a doctor before they get extremely sick.

6. Reduced exposure to pathogens

Hours-long waits in doctors’ waiting rooms with other patients can contribute to the spread of COVID-19, the flu, and other viruses. Telehealth keeps patients at home, avoiding exposure to viruses and germs. This helps protect medical professionals as well.

7. Middle-of-the-night care for babies and children

Babies have a knack for spiking fevers or getting sick in the middle of the night. Rather than rely on an internet search, parents can use telehealth services to connect quickly with doctors who can give answers and provide a diagnosis, and even a prescription, when needed.

8. No need for childcare

Parents get sick, too, and it can be challenging to take little ones along to a doctor’s visit. Telehealth eliminates this need, as well as the additional cost of childcare.

9. Doctors get to stay home too

During the pandemic, many medical offices have closed or reduced their hours. Cybersecurity tools such as virtual private networks have enabled doctors and therapists to treat patients safely from their home offices. This access also allows doctors and patients to connect after hours and on weekends.

White House coronavirus advisor Dr. Anthony Fauci urged young Americans to not take the coronavirus lightly, saying doing so could propagate the pandemic.

“You have to have responsibility for yourself but also a societal responsibility that you’re getting infected is not just you in a vacuum. You’re propagating the pandemic,” Fauci, director of the National Institute of Allergy and Infectious Diseases, told Facebook CEO Mark Zuckerberg in an interview Thursday evening.

The comment by Fauci came as state health officials say more young people are ignoring social distancing measures and contracting the virus at a higher rate. Fauci said the average age of a new Covid-19 patient has dropped by 15 years since the beginning of the pandemic in the U.S.

Vice President Mike Pence warned last month that roughly half of the new cases in the U.S. were people under the age of 35, particularly in Florida and Texas.

Fauci said young people at bars enjoying themselves is “totally understandable.” But he warned that young people who are asymptomatic, who never develop symptoms, can also pass the virus on to others.

Telemedicine termed a 'silver lining' of the coronavirus pandemic. This time, it might just be true.

Telehealth use surged from 8% of Americans in December to 29% in May as primary care, mental health and specialists turned to remote care out of necessity during the COVID-19 pandemic, according to a UnitedHealth Group report.

Telehealth evangelists long have touted using high-speed Internet connections and a range of devices to link providers and patients for remote care. But regulatory hurdles and medicine's conservative culturelimited virtual checkups to largely minor conditions like sinus infections or unique circumstances such as connecting neurologists to rural hospitals that lack specialized care.

The pandemic lockdowns closed doctors offices and delayed non-emergency care for millions of Americans. Some clinics scrambled to acquire technology platforms to deliver remote care. Others began employing rarely used video programs to reach patients in their homes.

Remote visits among Medicare patients surged through the end of March, prompting Centers for Medicare and Medicaid Services Director Seema Verma to say she "can't imagine going back."

Aided by emergency legislation that eased Medicare payment restrictions and allowed doctors to practice across state lines, some predict a significant portion of Americans will choose to get care remotely as stay-at-home orders lift.

Telemedicine Takes Lead In Cancer Care During COVID-19 Outbreak

Telemedicine has been implemented in medical practices across the country in response to “stay at home” orders and social distancing recommendations from local governments and health authorities. For some practices, telemedicine was already an extension of their day-to-day operations. For other practices, conducting patient visits via telephone or internet is a novel concept.

Oncology practices especially have to take careful steps for deciding which patient cases can be managed through telemedicine and which require in-person visits. Some examples of individuals who may be ideal for telemedicine visits are those who are finished treatment and only require surveillance. Other patients who may be cleared for telemedicine are those who have been responding to their treatment long-term.

“Telemedicine expanded greatly in a short time out of necessity, but with that we have seen how valuable it can be as an alternative to live visits for some patients. I believe that we will continue to employ telemedicine far beyond these acute issues,” H. Jack West, MD, told Targeted Oncology.

Two young U.S. doctors spend their honeymoon helping patients battle COVID-19

(Reuters) - She'd picked her dress, they'd booked their honeymoon and the wedding was set for the end of March. Then, the coronavirus pandemic hit.

So the two young American doctors, Kashif Chaudhry and Naila Shereen, whose whirlwind courtship spun them between New York City and Cedar Rapids, Iowa, cancelled their big plans.

Two weeks ago, they persuaded the imam at a mosque in Hawthorne, New Jersey, to open up for a quick nikkah marriage contract ceremony. The couple married the following day on Saturday and celebrated with her family at her parents' house in New Windsor, New York. After the ceremony, Shereen dropped her new husband at the airport 12 hours later.

"We said our goodbyes - we were a bit teary and sad," he said. "I gave her a red rose."

USC expands telehealth to keep patients & workers safe amid coronavirus pandemic

Going to a hospital or clinic can be scary for people who need to see a doctor but are worried about the growing coronavirus pandemic.

Thankfully, many of those seeking care from doctors, nurses and other health care workers at Keck Medicine of USC have another option: a video visit from the comfort of their home.

All health care providers with Keck Medicine’s hospitals and clinics throughout Southern California are now credentialed to practice telemedicine. They’ve checked on thousands of patients during the past few weeks using videoconferencing technology, eliminating the risk of spreading COVID-19 to these patients through their practices.

“What is so heartwarming is when our patients can see their doctor and there is definitely still that connection,” said Laurie Johnson, chief ambulatory officer at Keck Medicine. “That smile and warm touch still comes across, even when it’s a telemedicine visit. Our patients are thrilled, and I see how our doctors are adapting to this, too. Through a stressful and difficult time, we’re seeing that humanity come through.”

It’s all thanks to a massive push by health care providers, information technology specialists and hospital and clinic administrators to expand telemedicine at USC. In only a month, what had been a small pilot project in a handful of clinics grew into a full-fledged system that now keeps Keck Medicine patients and health care workers safer.

“All these resources went into overdrive and rapidly rolled out at scale what we previously had been doing at a snail’s pace,” said Dilip Parekh, professor of clinical surgery and chief clinical officer for digital health and Keck Medicine’s provider network. “We went from something like 15 visits a week to almost 1,000 visits a week in a matter of a couple of weeks. It’s really a testament to teamwork.”

The global coronavirus pandemic has accentuated major failures in our healthcare system. The number of patients needing treatment for the virus itself is off the charts, while shortages in supplies and professionals raise fears that not everyone that needs treatment will get it. Simultaneously, social distancing and quarantine measures have forced many doctors to cancel routine and non-emergency appointments.

No one can say how long the virus will last and how it will ultimately impact on our lives and economy, but one thing is for certain—our healthcare system is not working, and we need to fix it.

While the country is divided on whether or not the government should provide healthcare for all, technology solutions that could lessen the strain of this pandemic, such as telemedicine, already exist, but there’s no real push to implement them widely. We know how to develop a secure communications system that will allow doctors to connect with patients from any location. We have the technologies to conduct remote diagnostics. Hospitals and doctor’s offices are increasingly using cloud platforms that allow them to store patient records, send bills, and provide communications between the patient and their doctor. Health trackers, such as the FitBit and Apple Watch, are already mainstream.

So, what exactly is holding us back? The real problem is a lack of participation by all stakeholders - including the insurance industry, hospitals, and clinics. They are comfortable with their existing business models and are reluctant to change their practices and allow access to much-needed technology solutions. In addition, current government regulations are not pro-market.

Telemedicine is having a moment. How can patients make use of the growing industry?

A mask and an iPad. That’s what a patient displaying symptoms associated with the novel coronavirus is handed when they arrive at Thomas Jefferson University Hospital in Philadelphia.

From an isolation room, the patient can then use their iPad to video chat with a remotely-located nurse practitioner who can evaluate their symptoms and determine next steps -- all without a face-to-face interaction. The process is called tele-triage, and it is just one of several ways telehealth is revolutionizing patient treatment in the coronavirus era.

From symptom-checking text bots to more traditional video chat consultations, telemedicine is having a moment -- and it’s a moment industry leaders say is a longtime coming.

"This crisis has supercharged interest and opportunity in telehealth on both the patient side and the provider side," said Dr. Ryan Arnold, an Omaha-based orthopedic surgeon at OrthoNebraska. "I feel like this is accelerating a trend that was going to happen regardless. And it’s going to be here to stay on the back end."

Medical professionals across the board agree that broad-scale telemedicine could help solve many of the problems confronting the American health care system as it struggles to stay afloat amid the crush of COVID-19 cases.

"What telehealth can do is help both the clinicians as well as the consumer have access to service without actually getting exposed to the virus," said Ann Mond Johnson, CEO of the American Telemedicine Association (ATA). "It mitigates the risk. So in that regard it’s incredibly valuable."

Physicians have already begun using telemedicine to communicate with patients remotely via video chat. Patients who do so are less likely to contract or transmit the disease than if they were to visit the doctor’s office in person. Dr. Danielle DeHoratius, a dermatologist in Pennsylvania, began consulting patients this way soon after the coronavirus outbreak. One of her patients contacted her about a rash around his eyes, and said he had come into contact with the virus.

The FDA is temporarily easing restrictions on veterinarians to allow them to more easily use telemedicine to treat pets during the coronavirus pandemic.

The FDA will suspend its rules requiring vets to physically examine pets, allowing them to prescribe drugs with a video examination.

Telemedicine has played a big part in health services during the outbreak.

The U.S. Food and Drug Administration said Tuesday that it’s temporarily easing restrictions on veterinarians to allow them to more easily use telemedicine to treat pets during the coronavirus pandemic.

“We need to provide veterinarians with the latitude to expand the use of telemedicine in the care of animals, not only pets but also the animals that produce our food,” FDA Commissioner Stephen Hahn said in a statement.

The Trump administration and public-health officials are urging consumers to use telehealth services to get remote treatment, fill prescriptions and get medical attention during the new coronavirus pandemic, and companies that offer virtual appointments are reporting a surge in demand.

The goal is to keep people with symptoms at home and to practice social distancing if their condition doesn’t warrant more intensive hospital care. Almost 80% of hospitals in the U.S. have some sort of telehealth service.

The new emphasis is designed to free up doctors and other front-line providers by encouraging patients with unrelated ailments such as diabetes to get care online.

Doctors & Patients Turn To Telemedicine During Coronavirus Outbreak

The use of virtual visits climbs as a way of safely treating patients and containing spread of the infection at hospitals, clinics and medical offices.

The man had recently traveled, including a brief stop in Tokyo. He had a fever and cough about a week ago, but was now feeling fine.

He called the virtual medical line set up by Rush University Medical Center in Chicago recently to help screen patients for coronavirus.

“He said all the right buzzwords: cough, fever, fatigue,” said Dr. Meeta Shah, an emergency room physician at Rush.

After talking with him, Dr. Shah did not think he needed to be admitted but referred him to the city’s health department.

Rush and other large hospitals across the country are quickly expanding the use of telemedicine to safely screen and treat patients for coronavirus, and to try to contain the spread of infection while offering remote services.

“This is a kind of turning point for virtual health,” Dr. Shah said. “We’re actually seeing how it can be used in a public health crisis.”
